Can anyone help my find this for a birthday present?

Hey BCB, I need your help. I am not finding it anywhere on the Web. Wanted for sentimental reasons, and there is a long story that goes with it. If I had the disposable cash right now, I’d buy one, rip or burn off anything that might be associated with the Cubs franchise, leaving only the name and number. I would then scrawl “SUCKS!” in acid green marker diagonally across the number. If I folded it right, I could probably get him to autograph it for me.
